Sexual Assault/Sexual Harassment (SASH) Incident Form

This form is to be completed by any member of the NTI community who experiences sexual harassment or sexual assault. This form
should also be completed by members of the NTI community who witness or become aware of incidents of sexual assault or sexual

SEXUAL HARASSMENT – is any unwelcome behaviour of a sexual nature that makes a person feel offended, humiliated or intimidated, in circumstances in which a reasonable person would have anticipated the possibility that the other person would be offended, humiliated or intimidated. It can be obvious or indirect, repeated or one-off. It can be physical or verbal and it can happen in person, over the phone or online.

SEXUAL ASSAULT – occurs when a person is forced, coerced or tricked into sexual acts against their will or without their consent. Sexual assault is a crime and is not the victim’s fault. It includes rape, indecent assault, any sexual contact with a child, forcing someone to witness a sex act, sexual act of touching that occurs after consent has been withdrawn.

CONSENT – in the context of sexual activity, it is free and voluntary agreement to engage in the specific sexual activity. Consent can be withdrawn at any time. The absence of “no” does not mean “yes”.

Any member of the NTI community who experiences, witnesses or becomes aware of sexual harassment or sexual assault is strongly encouraged to report the incident via this form. Reports by any person will be treated with confidentiality at all times.
